I just watched the commercial.
1)Yes, there is a bronze/gold trophy/statue of Wilt in his current "broken" state playing basketball (and honestly I wouldn't worry too much about whether it was made because he trumped a handicaps-only basketball tournament; it's just a statue saying he won a match, duh. He's awesome with basketball anyway.

2) The thing Wilt picks up at the end does really look more like a "prosthetic arm" than a fork, since it appears to have a "thumb" appendage. I'm looking at this from an animation standpoint and that Wilt picking up something that looks like an old arm of his is merely a sight-gag/cute joke and leaving it at that, lol. It was a nice, effective commercial.
I love the fact that the sales banner in the beginning reads "BLOO BROKE ALL THIS STUFF! EVERYTHING MUST GO!" XD
